Lunar Root Collective provides nutrition education and lifestyle support for fertility, pregnancy, postpartum, and early childhood nourishment. Services are focused on food, nourishment, and supportive wellness practices intended to complement—not replace—medical care.
We do not diagnose, treat, cure, or prevent disease, nor do we provide medical advice. Nutrition services are educational in nature and are designed to support overall wellbeing.
Clients are encouraged to continue working with their physician, midwife, or other regulated healthcare providers alongside nutrition support.
2. Professional Title & Transparency
Professional Credentials
Lunar Root Collective is operated by a Nutrition Consultant / Nutrition Practitioner (not a Registered Dietitian). The title “nutritionist” is not a regulated healthcare designation in Ontario.
Services provided do not fall under the scope of practice of regulated health professions and are offered for educational and supportive purposes only.

I agree to disclose relevant health information honestly and completely.
I understand that I may discontinue services at any time.
I consent to receive nutrition education and lifestyle guidance as described above.

This information is collected solely for the purpose of providing nutrition services and is stored securely using HIPAA- and PIPEDA-compliant platforms (including ClinicSense and Google Workspace).

9. Cancellation & Refund Policy
Cancellation Policy
Appointments must be cancelled or rescheduled at least 24 hours in advance. Late cancellations or missed appointments may be subject to a fee.
Refunds are not provided for completed services. Packages and programs are non-refundable once initiated, unless otherwise stated.
